Over a four-month observation period-January through April 2026-the community achieved a 70% reduction in falls compared to its historic baseline, culminating in zero recorded falls in April 2026-the lowest monthly fall count in the community's history. In parallel, Helpany's AI-powered service plan assessment revealed that 1 in 3 care plans required realignment, meaning documented care levels no longer fully reflected residents' evolving needs and risks.

Falls remain one of the most dangerous and costly challenges in long-term care. Each year, more than 3 million falls occur in long-term care- and an estimated 1 million are preventable. The improvement strengthened month-by-month. January showed a meaningful reduction in falls compared to the 2025 baseline. February improved even further.
